Welcome to Elite EndoMet

Visit information for new and follow-up patients.

Your first visit is an opportunity to discuss your concerns, review your history, and begin a personalized plan for your endocrine and metabolic health.

Before your appointment

Three steps help us prepare for you.

Confirm the appointment details, complete the forms supplied by our office, and arrange for relevant medical records to arrive before your visit.

Confirm coverage and forms

Confirm your insurance benefits and any referral requirements. Complete the forms provided by our office, including the weight management intake if relevant to your visit.

Send pertinent records

Ask your previous clinicians to send pertinent records to fax 888-707-2525. Please share our practice name and address so the records reach the correct office.

Referral information

What to bring

Bring the information that completes the clinical picture.

If you are unsure whether something is relevant, bring it or call the office before your visit.

  • Photo identification and current insurance cards.
  • Your medication list with doses, allergies, vitamins, and supplements.
  • Relevant laboratory results, specialist notes, imaging, and pathology reports.
  • Any required referral or authorization information.
  • A short list of your main questions and goals.
Diabetes devices

Bring your glucose meter, CGM, or insulin pump.

Bring the device and any available reports if you use these technologies.

Condition-specific records

For osteoporosis visits, bring available bone density reports and fracture or treatment history. For thyroid, pituitary, or adrenal concerns, bring relevant prior imaging and testing.

What to expect

A thoughtful review, followed by clear next steps.

Understand your history

Dr. Edem will review your medical history, symptoms, prior treatment, and concerns. Evaluation may include an examination, discussion of existing results, and additional testing when appropriate.

Plan what happens next

We will explain the next steps and discuss treatment options. Some diagnoses and treatment decisions require additional information and may be finalized at a follow-up visit.

Preparing for testing

Do not change your routine solely because you have an appointment.

Do not fast, stop medicines, or change supplements unless you have specific instructions from our office or the clinician ordering your tests. Call if the instructions are unclear.
