This site is not a patient portal. Use only the communication routes provided by the practice for clinical information or medical records.

The appointment request form asks for contact information, patient status, the general type of care requested, communication preference and limited scheduling details. The form is processed directly through the practice’s Google Workspace account rather than stored in the website’s Cloudflare database.
The site may record limited first-party counts when visitors select an appointment, telephone or referral link. These event records are intended to measure general website use and do not include names, email addresses, telephone numbers or medical details.
Do not submit detailed symptoms, diagnoses, medication lists, insurance identification numbers, medical records or other sensitive clinical details through the appointment form or ordinary email. The office will explain the approved route when those details are needed.
How information is used
Appointment request information is used to contact you about scheduling and administrative next steps. Limited website activity information may be used to understand general site use, secure and maintain the website, improve access to practice information and meet legal obligations.
Sharing and retention
Information may be processed by service providers used to host, secure or operate the website and appointment workflow, including Google Workspace. We do not sell information submitted to the practice. Information is retained only as long as reasonably necessary for its purpose, legal compliance, security or dispute resolution.

You may request an appointment by telephone instead of using the online form. You may ask a privacy question by calling 844-363-6638, subject to legal and operational requirements.
Security and children
Reasonable safeguards are used, but no internet transmission is guaranteed secure. This website is intended for adults and is not knowingly directed to children under 13.
