Medical records release

Request your records or arrange a transfer of information.

Access to relevant medical records helps support coordinated care. Our office can explain how to obtain a copy of your Elite EndoMet records or arrange for records to be sent to another clinician or authorized recipient.

Choose the right request

Start with where the records need to go.

Requests for your own copy and authorizations for disclosure to another recipient follow different processes. We can help you identify the appropriate option.

Request a copy for yourself

Get the records you need in the format available to you.

Call 844-363-6638 for instructions. We may ask you to complete a records request and provide information needed to verify your identity.

  • Specify which records you need
  • Include the relevant date range
  • Tell us your preferred delivery format
Our office can explain available options and any applicable copying charges before processing the request.

Send records to another person or organization

Use the appropriate authorization when one is required.

Contact our office for the appropriate authorization. Include the recipient's name and contact details, the information to be released, the purpose, and the required signature and date.

Two distinct processesPatient access requests and authorizations for other disclosures are different. Our team can help identify which applies.

Transfer between treating clinicians

Coordinate information for continued treatment.

Records may generally be shared between clinicians for treatment without a separate HIPAA authorization, although additional requirements can apply to certain information.

Contact our office to coordinate the transfer

Bring records from your previous practice

Send the clinical details that help complete your history.

Ask your previous clinician to send relevant records to Elite EndoMet. Records from Dr. Edem's previous practices may need to be requested directly from those organizations.

  • Relevant office notes
  • Laboratory results
  • Medication history
  • Imaging reports
  • Pathology reports

Submit a request or ask for help

Arrange a secure submission method before sending records.

Call our office for the approved submission method. Do not send sensitive records through a general website form or ordinary email without arranging a secure method.

Requests are processed under applicable requirements. If records are needed for an upcoming appointment, include the appointment date so our team can discuss coordination.
