What are your office hours, and how can I schedule an appointment?
Office hours are Monday through Friday, 8:00 a.m. to 4:00 p.m. Request an appointment by calling 844-363-6638, completing the appointment form on this website, or asking your primary care or specialty clinician to send a referral. Our fax number is 888-707-2525.
Do I need a referral to see Dr. Edem?
Not always. Referral requirements depend on your insurance plan and clinical situation, so please contact your insurer and our office before scheduling. If a referral is needed, it may come from your primary care clinician or a specialist. Ask the referring office to include your current problem list, medication and allergy lists, relevant imaging, and most recent laboratory results.
Are you accepting new patients?
Yes. Elite EndoMet is preparing to welcome new patients beginning November 16 for endocrine, diabetes, obesity, and metabolic care. For adults age 18 and older only. Contact our office to confirm appointment availability and scheduling requirements.
What insurance plans does Elite EndoMet accept?
Elite EndoMet accepts Medicare and Medicare Advantage and is completing credentialing with major commercial insurers. Blue Cross Blue Shield participation is pending approval. We do not accept Medicaid, although self-pay options are available. Because participation varies by plan and network, please confirm coverage with our office and your insurer before your visit.
Do you accept self-pay patients, and how much does a visit cost?
Yes. Our standard self-pay rates are $400 for a new-patient visit and $200 for a follow-up visit. A limited-time introductory rate reduces these to $200 for a new-patient visit and $100 for a follow-up visit. Laboratory testing, imaging, medications, devices, and other services may be billed separately. Contact our office for current rates and a good-faith estimate when applicable.
Will my insurance cover endocrinology and obesity medicine visits?
Many plans cover medically necessary endocrinology services, but coverage for obesity medicine, weight-management visits, medications, and related services varies. Copays, coinsurance, deductibles, exclusions, prior authorization, and medication coverage depend on your plan. We recommend checking these benefits before your appointment.
What should I bring to my first appointment?
Bring a photo ID, current insurance card, medication and supplement list, and relevant medical records. If available, also bring recent laboratory and imaging reports, glucose meter readings, CGM information, and insulin pump data. Having these records helps us complete a more thorough assessment.
What can I expect during my first visit, and how long will it take?
Your first visit generally includes a detailed medical history, review of condition-specific intake forms, discussion of symptoms and medications, evaluation of prior testing, and conversation about your goals. Dr. Edem will explain relevant findings and treatment options and develop an individualized care plan. Visit length depends on the complexity of your concerns.
Will I see Dr. Edem personally at each appointment?
Yes. Elite EndoMet is Dr. Edem's independent solo practice, so you will see Dr. Edem at each clinical appointment.
Do you offer telehealth or virtual follow-up appointments?
Telehealth may be appropriate for certain follow-up visits when clinically suitable and allowed by your insurance plan and applicable regulations. Some concerns require an in-person examination or testing. Telehealth is not available for new-patient visits.
Can I transfer my care from another endocrinologist to Elite EndoMet?
Yes. We welcome patients who wish to establish care with a new endocrinology practice. Please arrange for relevant office notes, laboratory results, imaging, and medication information to be transferred before your visit to support continuity of care.
How do I access laboratory results, medical records, and the patient portal?
Eligible patients may access available health information through our secure Healow patient portal. Our office can also explain how to request medical records or obtain copies of results.
How do I request prescription refills, and how long do refills take?
Request routine refills before your medication runs low through the approved office or patient-portal process. Some prescriptions require laboratory monitoring or a follow-up visit. Please allow 48 to 72 business hours for non-urgent refill requests.
How will I receive laboratory results, and will someone explain abnormal results?
We strive to review and respond to routine laboratory results, imaging results, and patient messages within 48 to 72 business hours of receiving them, through the secure portal or by phone. Urgent findings are prioritized. Weekends, holidays, office closures, and results requiring additional review may extend this timeframe. Call the office for urgent concerns and call 911 for a medical emergency.
What is your cancellation, rescheduling, and no-show policy?
Please provide at least 48 business hours' notice to cancel or reschedule. Late cancellations and missed appointments may incur a $50 fee. Patients arriving more than 15 minutes late may need to reschedule. Genuine emergencies and unavoidable circumstances are considered case by case.
How do I contact Elite EndoMet after hours or for urgent medical concerns?
For life-threatening emergencies, call 911 or seek emergency care. For non-emergency concerns, contact our office during regular business hours. Our team will explain the appropriate process for time-sensitive questions and follow-up needs.
